Highest completed difficulty in two separate screens to be the same.

How often does this happen? When did it begin happening?
This is the case through the bank on all dungeons I have. Needless to say the actual completed difficulty is X. The last update had this wonky as well. It does appear though that the loot ticket use acknowledges X
