This is an Epic Firewalker’s Necklace with an abilty that supposedly procs at a 40% rate:
After extensive testing, the rate is more accurately 0%; in other words, it does not work. I have tested upwards of 50 Big Red Gem creations and not a single one upgraded as suggested by the Necklace.
Yep, tested it in a red generation build made for testing this item and I cant get it to fire either.
Really disappointing as I have this in my kingdom defense build - apparently pointlessly

Thank you for bringing this to our attention. I’ve tested this Firewalker’s Necklace quite a bit and I could get its ability to trigger too.

We are close to 1.5 now, can you tell if this will be fixed soon? I’ve collected enough glyphs and t4 relics to upgrade this necklace to mythic, but I don’t want to waste resources.
